The player can miss out on some content if they progress the game too quickly or through some choices. Here's a centralized list of such things
Progression:
Completing Act 1 (In Her Footsteps): Royal Amnesty, Cow Herding (Not a journal quest, see Livrea for more details)
Entering Khor'Minos: Mad Cow (The quest will be failed if in progress, or become unobtainable if not.) Evelyn if the player hasn't done her first encounter beforehand (May or may not be intentional.)
Completing Act 2 (The Siege of Khor'minos): Winter Wolf, Harboring a Fugitive
Choices: Mad Cow: Completing the quest locks the player out from Brint content. Failing the Quest locks the player out from Brienne content.
